Pls help me. I can't complete Zombie Slayer 1 and 4 for some reason.

As the title says, I can't achieve Zombie Slayer 1 and 4. I did beat Revenant Horror Tier 1 months ago. Since then, I haven't become Zombie Slayer 1 for some reason. However, when I beat Tier 2, Zombie Slayer 2 was unlocked, and I became Zombie Slayer 3. I don't do many Slayer quests, so I left 1 uncompleted, but today, when I tried to craft a Slayer item, I was able to craft Zombie Slayer 2 and 3 items, but I couldn't craft only 1 because I hadn't achieved 1. I thought it was a bug, so I beat Revenant Tier 1 again, but it still didn't work. Then, when I became Zombie Slayer 4, this bug occurred again. The recipe for Zombie Slayer 4 was not unlocked. But it definitely says LVL 4! This is terrible.
As you can see in the image below, recipes 2 and 3 are unlocked, but recipes 1 and 4 are not.
Is there a way to fix this?

Go to Maddox and "claim" the level. You get a chat message when you reach the level, but you don't unlock the rewards from that level until you physically claim them.
